Cumulative discount card

Our center operates a cumulative system of discounts with the subsequent issuance of a club card of the “Open Clinic” network (valid in all centers of the holding: Presnensky Center, Kuntsevsky Center under the scientific direction of V.I. Dikul, diagnostic and treatment center “Kutuzovsky”, International Surgical Center) . A discount card is issued to patients when paying for medical services in the amount of:

The discount card is not personal; while retaining all the benefits, it can be used not only by close relatives (upon presentation of an identification document - a passport or birth/marriage certificate), but also by friends by entering their contacts (full name, email, mobile phone number) in questionnaire with the condition of mutual consent.

Discounts on laboratory tests do not apply to discount cards.

When a higher discount level is reached, its percentage increases to the next denomination. All cards previously issued to clients in any of our centers are subject to exchange for a general card of the Open Clinic network upon their next application. In return, you are given a card with a similar basic discount. If there is no similar discount percentage in the new system, an increased percentage is charged to the new card in favor of the client.

The discount card is the property of the Open Clinic holding. We reserve the right to reissue, replace, cancel and suspend validity, as well as change the size and conditions of discounts on discount cards. The validity period is unlimited.

Discounts for schoolchildren
and students

For schoolchildren, university students and technical school students of any form of education, our medical center offers a 10% discount on all types of services. To receive this type of discount, it is enough to present a valid student card or certificate of study at the time of payment for services at the registration desk.

Discounts
pensioners

For pensioners at the Medical-Surgical Center there is a 10% discount on all types of services. To receive a discount, you just need to present your pension certificate. Discounts cannot be combined and do not apply to laboratory diagnostics.

POSITION ON THE DISCOUNT SYSTEM IN LLC MC "EN CLINIC"

Discount card is a plastic card that provides the opportunity to receive discounts when using services at LLC MC "EN CLINIC" in the amount and on the conditions stipulated by these Regulations. The discount card is the property of LLC MC "EN CLINIC" and is issued only at LLC MC "EN CLINIC" on the Company's terms. Discounts provided on the Discount Card are determined by the administration, depend on the amount of savings and may be changed by the Company’s decision.

To receive a discount card you must:

Verbally declare to the administrator of the Medical Center your desire to issue a Discount Card;
- fill out a personal form and give consent to the processing of personal data;

Agree to the Terms of Use of the Discount Card.

The buyer's refusal to fill out the questionnaire is grounds for refusal to issue a Discount Card.

When a Discount Card is issued, its validity does not apply to the transaction on the basis of which it is issued.

1. A discount card is not a credit, payment or bank card and does not require additional activation.
2. The discount card is not personal.
3. The discount card is kept by the patient; each has its own unique barcode and individual number indicated on it.
4. One person can only own one discount card.
5. The discount card of LLC MC "EN CLINIC" is cumulative for the amount of services provided. The amount accumulates on the patient’s account starting from the date of card activation.
6. The card entitles the owner to take advantage of a discount when paying for medical services provided at LLC MC "EN CLINIC".
7. To receive a discount, the patient must present a Discount Card before making a payment for medical services. The discount system does not apply to patients who have not presented their Discount Card. The card is presented before or after the service is provided.
8. The value of the Discount Card increases when the following amounts are accumulated:

5% Discount card ( Discount ) is issued to everyone upon initial application or upon accumulation of an amount of up to 9999 rubles;

10% Discount card ( Silver ) is issued upon accumulation of an amount from 10,000 to 19,999 rubles;

15% Discount card (Gold ) is issued upon accumulation of an amount from 20,000 to 34,999 rubles;

20% Discount card (Plat inum ) is issued upon accumulation of an amount from 35,000 to 49,999 rubles;

25% Discount card ( VIP ) is the last card when you accumulate an amount of 50,000 rubles or more.

1. Lost or unusable cards must be exchanged when contacting the administrator at LLC MC "EN CLINIC". After the patient has written an application, the administrator has the right to require an identification document (passport, driver’s license) from the card holder. After receiving a new card, the old discount card is blocked.

When the Discount Card is restored, the savings are saved.

When carrying out any companies or promotions that have discounts, as well as for preferential categories of citizens, discounts are not cumulative. The size of discounts and their duration may vary. The changes come into force from the moment they are approved by order of the Director of the Medical Center.

LLC MC "EN CLINIC" is obliged to inform patients who apply for paid medical services about possible discounts and promotions or changes to the conditions of previously provided discounts by posting information on stands and the official website.

LLC MC "EN CLINIC" has the right to suspend the Discount System at any time, of which the owners of the discount cards will be duly notified - by posting information on the official website and information stands at least 15 (fifteen) calendar days before the expected date of suspension of the discount systems.

1. General provisions.

1.1. LLC "Medical Center "Zemsky Doctor" (hereinafter referred to as the "Company, Organizer") issues its own Discount Cards, the owner of which is the Company.

1.2. In accordance with these Regulations, the Organizer of the Discount Program provides the opportunity to Patients who meet the requirements for participation in the Discount Program to join the Discount Program and receive incentives in the form of a discount upon presentation of the Discount Card.

1.3. Providing patients with discounts on services (products) provided through the use of Discount Cards is due to the following purposes:

• increasing Patient loyalty by encouraging them to receive and pay for medical services in cash or by bank card;
• ensuring high quality of the Company’s interaction with patients of all statuses and, on this basis, combining competitive prices;
• winning new and retaining old consumers;
• concluding a contract for the provision of services with the Patient on conditions that can be properly fulfilled by the Company and the patient.

The patient is provided with a discount on paid medical services determined by these Regulations.

2. Terms and Definitions.

2.1. Patient is any individual who receives services in cash or pays for services with a bank card, uses the services of Zemsky Doctor Medical Center LLC and has entered into an Agreement for the provision of medical services with Zemsky Doctor Medical Center LLC.

2.2. The organizer of the discount program is Zemsky Doctor Medical Center LLC, which provides medical services to Patients.

2.3. Discount program is a loyalty program of Zemsky Doctor Medical Center LLC for Patients served in cash or paying for services with a bank card, valid in accordance with these Regulations.

2.4. Discount program participant is an individual defined by the contract as a Patient who meets the requirements for participation in the Discount program and has confirmed his participation in the Discount program in the manner prescribed by the rules of the Discount program.

2.5. A plastic discount card with a company logo (with a face value of 3%, 5%, 7%, 10%) is issued to an individual and cannot be used to pay for medical services by legal entities. The discount card is used to record and provide discounts to the Discount Program Participant in accordance with the terms of the Discount Program. The discount card is not personal. Each Discount Card has its own unique number and is personalized.

2.6. Discount is the amount by which the price of a medical service is reduced for the Patient when paying for the medical services of the Organizer of the discount program.

3. General rules for using the Discount Card of Zemsky Doctor Medical Center LLC.

3.1. The condition for receiving the Discount Card of LLC "Medical Center "Zemsky Doctor" is that the patient makes a payment for services rendered in the amount of 10,000 rubles or more at LLC "Medical Center "Zemsky Doctor".

3.2. The discount card is issued to individuals free of charge, subject to the purchase of medical services at Zemsky Doctor Medical Center LLC. Upon receipt of the card, the Patient is required to sign for receipt, which means the Patient’s agreement with these Regulations.

3.3. The discount card is the property of the Discount Program Organizer.

3.4. Any individual who has entered into an agreement for the provision of medical services with Zemsky Doctor Medical Center LLC can become a participant in the Discount Program. The discount card of LLC "Medical Center "Zemsky Doctor" is a confirmation of the right of the bearer of this card to demand a discount, on the one hand, and the obligation of the Company, during the validity period of the card, to provide the card holder with a discount equal to the face value of this card when purchasing a service on the day of concluding the contract for the provision of services, on the other side.

3.5. A discount card can be obtained by accumulating amounts of 10,000 rubles, 20,000 rubles, 40,000 rubles, or 70,000 rubles or more when the Patient pays for medical services for the entire period of service at Zemsky Doctor Medical Center LLC, which has entered into a service agreement with the Company.

According to the savings, discount cards of different denominations are issued.

• When you accumulate 10,000, a card is issued with a 3% discount.
• When you accumulate 20,000, a card is issued with a 5% discount.
• When you accumulate 40,000, a card is issued with a 7% discount.
• When you accumulate more than 70,000, a card is issued with a 10% discount.

3.6. The amount of payment made for services (goods) provided is taken into account in the discount program of Zemsky Doctor Medical Center LLC automatically when paying for services.

3.7. You can find out about the current amount of savings on the Discount Card at the reception of Zemsky Doctor Medical Center LLC.

3.8. It is not possible to use the card to receive funds. The discount amount is not given in monetary terms. When paying for medical services, only one Discount Card can be used. Using the Discount Card means unconditional agreement with the rules for its use presented in these Regulations.

3.9. The provision of a discount to the Participant of the discount program is carried out automatically upon presentation of the discount card at the reception of the Organizer of the discount program at the time of payment for medical services.

3.10. The organizer of the discount program has the right to refuse to provide a discount if the patient does not present a Discount Card at the time of payment for services.

3.11. The Company has the right to unilaterally, without prior notice to the Discount Card Owner, cancel the Discount Card if:

• the owner of the Discount Card violates the rules for using the Discount Card;
• it has been established that the Discount Card Owner committed fraudulent actions in relation to the Company’s property.

3.12. The organizer of the discount program reserves the right to change the rules and terms of the Discount Program without prior notice to the Participant of the discount program. The organizer of the discount program reserves the right to completely terminate the validity of discount cards at any time. Information about these changes is available on the Company’s website, as well as at the register of Zemsky Doctor Medical Center LLC, indicating the moment of entry into force. The owner of the Discount Card independently monitors changes in the terms of circulation of the Discount Cards.

3.13. Ignorance of these rules is not a basis for any claims by Discount Card Owners. The company is not responsible for direct or indirect losses associated with their use. The company guarantees the protection of personal data provided by the client during registration and data obtained when using cards.

3.14. The discount program participant who has joined the Discount Program accepts all the conditions offered by the Discount Program Organizer and consents to the processing of his personal data. The organizer of the discount program confirms that when processing personal data, he is guided by the legislation of the Russian Federation in the field of personal data, including the Constitution of the Russian Federation, international treaties of the Russian Federation, Federal Law No. 152-FZ “On Personal Data” dated July 27, 2006, other federal laws defining the cases and features of the processing of personal data.

4. Rules for providing discounts using the Discount Card of the NGO “Medical Center “Zemsky Doctor”.

4.1. The discount on the Discount Card is provided upon presentation by the Participant of the discount program at the registration desk of the Organizer of the discount program at the time of payment for medical services. When a discount is applied, the price of the medical service decreases.

4.2. Medical services participating in promotions may be subject to restrictions or special conditions for providing discounts using this Discount Card.

4.3. A discount on a Discount Card is provided only upon presentation of the card by the person in whose name the card is issued. To use the discount card, the Patient must present an identification document to the Medical Center Employee.

4.4. The discount card is not transferable to third parties.

4.5. The discount is not provided if the patient has a debt to Zemsky Doctor Medical Center LLC, or in case of incomplete (partial) payment for medical services.

4.6. The discount does not apply to:

• purchase of all types of comprehensive programs, gift certificates, treatment courses;
• services provided under a VHI policy, gift certificate;
• promotional goods and services;
• corporate service.

5. Restoration of the Discount Card.

5.1. If the Discount Card is lost or damaged, it may be replaced with another Discount Card, provided that the Discount Program Participant fulfills the following conditions:

• presentation by the Discount Program Participant, for whom the Discount Card was issued, of an identification document;
• existence of an agreement for the provision of medical services signed between the Patient and the Company.

5.2. In case of replacement of a discount card due to damage or loss and the Participant of the discount program does not have a discount card to make a replacement, a new Discount card is provided upon writing an application to the management of the Organizer of the discount program with a request for the issuance of a new Discount card and an explanation of the reasons for the inability to return earlier issued Discount Card. A write-off report is drawn up for a previously issued Discount Card based on the client’s application. The Discount Card is included in the list of lost and damaged Discount Cards.

6. Refund.

6.1. In case of a justified return of funds by the Organizer of the discount program for medical services that were not received by the Participant of the discount program or for medical services that were poorly provided to the Participant of the discount program or in other cases for which payment a discount was applied under this Discount Program, a refund is subject to amount, taking into account the discount provided.

7. Validity period of the Discount Card. Responsibility.

Final provisions.

7.1. The validity period of the discount card is unlimited.

7.2. All information regarding discount programs will be communicated to discount card holders by posting information on stands in the building of Zemsky Doctor Medical Center LLC and on the website pages.

7.3. A copy of the current Regulations, information on the current amounts of discounts must be in a visible place at the registrars and, at the request of the patient, made available for review. The text of these Regulations is posted on the website of Zemsky Doctor Medical Center LLC.

7.4. In the event of force majeure circumstances (including technical failure, loss of the database due to objective circumstances) and the failure to provide discounts on discount cards in connection with this, Zemsky Doctor Medical Center LLC is exempt from liability.
